Implementation Guide

Implementing Chorus within a healthcare organization typically involves a structured, multi-stage rollout that ensures the platform integrates effectively with existing clinical workflows and technical infrastructure.

System Setup

Configure access, user roles, and define clinical eligibility criteria.

Device Integration

Connect cellular monitoring devices and map to the FHIR Gateway.

Enrollment

Onboard patients, assign hardware, and provide initial training via the app.

Clinical Workflow

Care teams begin receiving and reviewing real-time telemetry.

Operations

Monitor program-wide KPIs and generate automated CPT billing exports.

Detailed Implementation Phases

1. System Setup

Initial setup includes configuring platform access via AWS Cognito, establishing role-based access control (RBAC), and defining organizational units. Healthcare organizations configure monitoring programs that define specific patient eligibility criteria, vital sign baselines, and clinical escalation workflows.

2. Device Integration

Monitoring devices are registered and connected to the platform payload parsers to begin transmitting physiological data securely over cellular networks. Device configuration ensures that incoming signals are properly parsed into standard FHIR Observation resources and associated with correct patient records.

3. Program Enrollment

Patients are enrolled into monitoring programs based on clinical eligibility and specific CarePlan requirements. Enrollment includes physical device setup, logistics tracking, and patient education to ensure proper device usage and adherence.

4. Clinical Workflow Integration

Providers and care teams begin reviewing monitoring data through the Chorus Provider Portal. Monitoring alerts, clinical findings generation, and patient communication workflows are firmly integrated into the provider's daily care operations, minimizing alert fatigue via the Agentic AI triaging.

5. Operational Monitoring

Healthcare organizations monitor overall program performance through administrative dashboards. These tools provide macro-level insight into patient participation, hardware connectivity, monitoring adherence, and automatically generated billing metrics for RPM CPT codes.
