Glossary

The following glossary provides definitions for commonly used terms within the Chorus remote patient monitoring platform and documentation.

A

Agentic Workflow
A system architecture that enables automated reasoning processes to analyze data, trigger workflows, and request human intervention only when needed.

B

Biosignal
Physiological data generated by medical devices, such as heart rate, blood pressure, glucose levels, oxygen saturation, or ECG waveforms.

C

Care Plan
A structured set of clinical actions and monitoring activities designed to manage a patient's health condition over time.

D

Diagnostic Report
A structured clinical summary generated from physiological data analysis and provider interpretation, commonly mapping to the FHIR DiagnosticReport resource.

E

ECG (Electrocardiogram)
A test that measures the electrical activity of the heart to detect rhythm abnormalities and other cardiac conditions.

F

FHIR (Fast Healthcare Interoperability Resources)
A healthcare data standard developed by HL7 that enables structured exchange of clinical information between systems.

FHIR Resource
A standardized data structure used within FHIR to represent clinical entities such as patients, observations, devices, and encounters.

O

Observation
A core FHIR resource used to store physiological measurements such as blood pressure, glucose readings, or ECG results.

R

Remote Patient Monitoring (RPM)
A healthcare delivery model that enables clinicians to monitor patient health data remotely using connected cellular medical devices.

Risk Classification
A categorization generated by the platform's analytical machine learning models indicating the level of potential clinical concern associated with patient data.

U

US Core Profiles
A set of implementation guidelines built on FHIR R4 that standardize how healthcare systems represent common clinical data elements within the United States.
