Data Model and Relationships

The Chorus data model is structured around standardized FHIR resources that represent clinical entities and relationships.

Key relationships within the system include:

  • Patient → Observation
  • Patient → Encounter
  • Observation → Device
  • Encounter → DiagnosticReport
  • Patient → CarePlan

Physiological readings collected from monitoring devices are stored as Observation resources that reference both the patient and the device that generated the data.

Clinical findings and provider interactions are represented using Encounter and DiagnosticReport resources.

This structured data model ensures that monitoring data remains organized, traceable, and interoperable.
